The only US medical school where fit is a legal status, not a preference. Every matriculant commissions as an officer in the Army, Navy, Air Force or US Public Health Service before starting, so applicants must be US citizens, meet the physical and security requirements for a commission, be at least 18 and no older than 36 as of June 30 of the matriculation year, and give evidence of a strong commitment to serving as a uniformed medical officer. The school states selection is based on motivation and dedication to a uniformed services career alongside an overall appraisal of personal and intellectual characteristics. Leadership is weighted far above the national norm here because the school describes itself as the leadership academy of military medicine. Roughly 60% of accepted students have no prior military service, so prior service helps but is not expected.
Watch out: The eligibility rules are absolute: non-citizens, applicants over the age cap, and anyone who cannot meet commissioning physical or security standards are ineligible regardless of academics. There is no tuition to weigh — students are paid officers — but there is a multi-year active-duty service obligation after residency, and ambivalence about it is disqualifying on the school's own stated criterion.
